We consider your health history, current medications, nutrition, movement, sleep, stress and personal goals. Care is led by Celeste McFarland, MSN, FNP-BC, PMHNP-BC, a dual board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner.

$100 initial consultation · 30 minutes
$50 follow-up · 15 minutes
Start with one appointment to review your goals, health history, relevant risks and next steps for an individualized plan.
Weight & Wellness Foundations
$299 / 12 weeks
One 30-minute initial evaluation and three 15-minute follow-ups. Includes a written nutrition and activity plan, review of relevant available health information and medication discussions when appropriate. An individualized Fullscript plan may be included when useful.
Weight & Wellness Plus
$449 / 12 weeks
One 30-minute initial evaluation, four 15-minute weight-management follow-ups and one separate 30-minute wellness/laboratory review. Includes an individualized lifestyle plan and optional supplement recommendations when appropriate.
Medication and Other Costs
When clinically appropriate, we discuss FDA-approved medication options, benefits, side effects, costs and follow-up needs. An appointment does not guarantee a prescription, testing or a particular result. Medications, pharmacy charges, laboratory fees and supplements are separate. Supplements are optional.

These services are private pay and available to adults 18+ in Illinois, Florida, Minnesota, Colorado, Arizona, Nevada, New Mexico, Utah, Delaware, Connecticut, Vermont and Washington. You must be in an eligible state for your visit.
