Weight & Wellness Foundations · $299 / 12 weeks
For adults 18 and older. Includes one 30-minute initial evaluation, three 15-minute follow-ups, a written nutrition and activity plan, review of relevant available health information and medication discussions when appropriate. An individualized Fullscript plan may be included when useful.

Included Services and Separate Costs
Fees cover the listed clinical visits and planning. Medications, pharmacy charges, labs and supplements are separate. Supplements are optional. Prescriptions, tests and outcomes are not guaranteed.
Before Your Appointment
You must be located in an eligible state at your visit: Illinois, Florida, Minnesota, Colorado, Arizona, Nevada, New Mexico, Utah, Delaware, Connecticut, Vermont and Washington. Payments are nonrefundable. Cancellations require at least 24 hours before your appointment. Use the secure scheduling/intake system for health information.
