Includes one 30-minute initial evaluation, four 15-minute weight-management follow-ups and one separate 30-minute wellness/laboratory review. You receive an individualized lifestyle plan and optional supplement recommendations when appropriate.

Separate Costs
Medications, pharmacy charges, laboratory testing and supplements are separate. Supplements are optional. No prescription, test or outcome is guaranteed.
Before Your Appointment
You must be located in an eligible state at your visit: Illinois, Florida, Minnesota, Colorado, Arizona, Nevada, New Mexico, Utah, Delaware, Connecticut, Vermont and Washington. Payments are nonrefundable. Cancellations require at least 24 hours before your appointment. Use the secure scheduling/intake system for health information.
