In CharmHealth, choose INITIAL Therapy Intake (Adult) for adults or INITIAL Therapy Intake (Pediatric) for ages 12–17.
Intake: 45–60 minutes · $225 private pay.
Follow-up: 45–60 minutes · $150 private pay.
Participating insurance may cover eligible services. Verify your specific plan before scheduling. Contact the practice for follow-up appointments or help finding an appropriate visit.

Before Your Appointment
You must be located in an eligible state at your visit: Illinois, Florida, Minnesota, Colorado, Arizona, Nevada, New Mexico, Utah, Delaware, Connecticut, Vermont and Washington. Payments are nonrefundable. Cancellations require at least 24 hours before your appointment. Use the secure scheduling/intake system for health information.
