Choose Your Intake
In the secure scheduler, choose INITIAL Adult Psychiatric Evaluation (medication management) for adults, or INITIAL Pediatric Psychiatric Evaluation (medication mangement) for patients ages 12–17.
Intake: 45–60 minutes · $225 private pay. The current online intake slot is 45 minutes. Follow-up visits are 20–30 minutes and $120 private pay. Participating insurance may cover eligible visits.

Before Your Appointment
You must be located in an eligible state at your visit: Illinois, Florida, Minnesota, Colorado, Arizona, Nevada, New Mexico, Utah, Delaware, Connecticut, Vermont and Washington. Payments are nonrefundable. Cancellations require at least 24 hours before your appointment. Use the secure scheduling/intake system for health information.
